The official source

Ayushman Bharat cards are issued under the PM-JAY scheme run by the National Health Authority. Beneficiaries can check their details and download cards on the beneficiary portal, and the NHA website explains the scheme. Save the card as a PDF or an image, then upload it here.

What the card shows

The card shows the beneficiary's name, the card or ID number and a QR code. If a photo is on your file, check that it is sharp. A QR code only prints well from a sharp file. Do not take a photo of a computer screen. Download the file again, or use the largest image you can get.

What to bring to the hospital

If you plan to use your Ayushman Bharat card, take it with you, along with your Aadhaar and the details of your family members covered under the scheme. Empanelled hospitals usually have a helpdesk, often staffed by an Ayushman Mitra, who can guide you. Each hospital applies the scheme's rules. A printed card is a convenience, and the helpdesk will tell you what else they need.

What the card does not do

The card does not enrol you in the scheme. Eligibility and treatment are decided under the scheme's own rules, and the hospital will tell you what it needs. We are not the National Health Authority. The PVC card is a printed copy of the file you give us. It does not check your eligibility or replace the official card.
